Case Description

The patient is a 64-year-old man admitted due to chest pain, which has been gradually worsening for the past 3 months. He described the pain felt like pressure in his mid-chest and mainly occurred on exertion and rated at 7/10/ The was no radiation of the pain to and was attenuated by rest. He has had no episodes of palpitations. He has been under treatment for coronary artery disease, hypertension, hyperlipidemia, diabetes mellitus type II, depression, and obesity.  His medications include insoluble insulin, lorazepam, nortriptyline, cholecalciferol, amlodipine-benazepril, aspirin, clopidogrel, esomeprazole, irbesartan-hydrochlorothiazide, nebivolol, and simvastatin. He has no history of smoking but takes an average of 2 beers a night. He has a history of hypertension and myocardial infarction in both his parents.

On examination, the blood pressure is 146/58 mmHg, the pulse 82 beats/minute, temperature 98.6oF, respiratory rate, 20 breaths/minute, oxygen saturation at 97%, height 6’2″, weight 285lb9.6 oz and BMI at 36.67 kg/m2. The S1 and S2 were normal. There was 2/6 systolic crescendo- decrescendo murmur was [resent in the right 2nd intercostals area, no gallops or run.

Diagnostic

The chest X-ray showed he had stable cardiomegaly. There were no remarkable mediastinal contours. There was no pulmonary infiltration nor pleural effusion. The echocardiogram showed the estimated ejection fraction at 50%. The aortic valve showed moderate focal thickening and calcification. The cusp separation was markedly reduced, showing severe stenosis and dilation of the aortic root. Also, the left atrium was dilated. Coronary angiography confirmed the diagnosis of left anterior descending artery stenosis.

Treatment

The patient was admitted for close monitoring. He was maintained on his antihypertensive medications and the statin.  The antihypertensive should be used cautiously to avoid an excessive decrease in the preload as well as the systemic arterial blood pressure (Mike, 2019).

He was scheduled for Aortic Valve replacement, which was done a day later.  He was put on warfarin to prevent clot formation. The surgery was successful, and the patient was scheduled for the first follow up the clinic after 2 weeks.  For every visit, a physical examination and an echocardiogram showed significant improvement in the patient. He P.T. and INR were checked on every follow-up visit as per the treatment guidelines for treatment on warfarin (Mike, 2019).
